Accident, Critical Illness and Hospital Gap Insurance are voluntary benefits and are administered by United Healthcare.
Accident Insurance
Summary: provides you and your dependents, if enrolled, with a lump-sum payment after an accident. Payments made to you from the accident plan can help with your out-of-pocket expenses, such as your deductible, co-pays, transportation for medical care, childcare and more. There are no coordination of benefits with your medical plan. Coverage is guarantee issue and pre-existing conditions are excluded.

Critical Illness Insurance
Summary: provides you and your dependents, if enrolled, with a lump-sum payment when diagnosed with a critical illness (as defined in the benefits summary). Payments made to you from the accident plan can help with daily living expenses, childcare, hiring household help, mortgage payments, college tuition, and treatment not covered by your medical plan. There are no coordination of benefits with your medical plan. Coverage is guarantee issue and pre-existing conditions are excluded.
Cost: enroll with either a $20,000 policy. Enrolled spouse will have a $10,000 policy and each child enrolled will have a $5,000 policy.
