The Annual Personal Health Assessment (PHA) event will be held Monday, July 18, 2022 to Tuesday, July 19, 2022. Registration is open. Please reference this flyer for full information regarding the PHA event.

What is a PHA?

A free biometric screening that includes a check of blood pressure, height, weight, waist measurement, and a blood draw to measure key lab values. Anyone who receives a PHA will be entered in to drawings for wellness prizes.

Location and Parking:

Training Room at the Bob Bennett Utilities and Transportation Facility (3400 Sunrise Rd.) Please park in the north parking lot that is accessible from Sunrise Rd (please do not enter from Luther Peterson).

Who is eligible to receive a PHA?

All employees and dependents (age 18 and older) who are enrolled with the City’s health insurance.
